The Inquiry鈥檚 Phase 2 report was released this week, distilling 400 days of evidence and more than three hundred thousand documents. The report concluded the fire which killed 72 people was the culmination of 鈥渄ecades of failure鈥� by government and others in the construction industry. It set out a 鈥減ath to disaster鈥� stretching back to the early 1990s and criticised organisations at every level. It said companies in the industry were 鈥渄ishonest鈥� and that all deaths in the fire were 鈥渁voidable鈥� and that residents were 鈥渂adly failed鈥� by those responsible for their safety.
