Fiona Flaherty's husband died after a crash on the M4 near Slough in October.

Fiona Flaherty:

"It was eight weeks ago, it was Thursday 20th October - which was the Thursday of the week just before half term.

"We want to know what happened, it's not about apportioning blame. Nobody saw Richard moving into the central reservation...there were no skid marks.

"Somebody needs to come forward and tell us what has happened. We're a broken family."
