Five years after meltdown at Fukushima, caused by an earthquake and tsunami hitting Japan, the nuclear industry is still learning safety lessons.
We analyse how the world's nuclear power companies have reacted to the disaster and we hear the views of Lady Barbara Judge, a former head of the Atomic Energy Authority in the UK. She also advised the Japanese power provider TEPCO.
