Crewmates Dec and Nicole are urgently dispatched to a baby who has stopped breathing, and they also attend to a 12-year-old patient with a history of being bullied.

As the day shift starts for ambulance crews across Lancashire, there are reports of multiple road traffic incidents across the county. Crewmates Dec and Nicole are dispatched to one scene, where an eyewitness has watched a driver crash into railings. Meanwhile, Anthony and Bex are dispatched to a man who has fallen off a ladder. They are able to treat his medical injuries but are saddened to hear that he is still adjusting to life as a widow, and the pandemic has meant a delay in receiving any grief counselling for his loss.

Later, Dec and Nicole attend to a 12-year-old patient who is struggling with pain in her abdomen and vomiting blood. Due to her age, they are very concerned by the symptoms, and as the patient shares her experience of bullying, it appears there is much more to the story. Dec is able to offer some much-needed support, having experienced bullying himself. He has recently decided to transition from female to male, and his relationship with Nicole has remained as strong as when they first met.

As crews book on for the night shift, the entire service is shocked by the sad news that a student paramedic has taken their own life. They have little time to digest the news before an urgent 999 call reports that an eight-week-old baby has stopped breathing. In control, call handler, Tom tries to calm a caller who is struggling with voices in their head, and finds a unique technique to do so. Elsewhere, a patient is trapped in woodland with a reported dislocated knee. Senior paramedic Lee soon finds that the patient is trapped in an area impossible to reach by vehicle, so Lee is forced to make his way on foot with the patient's brother to guide him along the dark river bank. While Lee struggles to find his patient, Dec and Nicole are dispatched to a patient who is feeling suicidal and has been drinking heavily.
