Polly Ross was allowed to leave Westlands Mental Health unit in Hull to go and buy cigarettes. Within hours she had killed herself. Acoroner ruled yesterday that the decision to allow her out unchaperoned had beeen a "very serious failure" that "had a direct causal effect" on her death. Genna Rourke was a friend of Polly Ross. Dr Gillian Strachan is a consultant psychiatrist from the Royal College of Psychiatrists.
